In an open form that can be used on ordinary desktop computers and laptops, CarePrecise Access Complete delivers every data field, and every row of data, contained in the National Plan & Provider Enumeration System (NPPES) The Centers for Medicare and Medicaid Services (CMS) maintains this database. All healthcare providers who are HIPAA covered entities must obtain a National Provider Identifier (NPI) by reporting their information via the NPPES, and updating it when it changes. APPLICATIONS
CarePrecise Access Complete is a normalized relational database. The format is specifically designed to be used on standard office-class desktop and laptop computers, overcoming the filesize and maximum row and column issues posed by oversized databases, using a process that removes blank cells from flat files and organizes the data into separate tables for name information, address information, etc., plus data from other federal sources. This allows queries to act on the full (nearly 5 gigabyte) dataset within a normal PC environment, either using the Microsoft Access mdb files, or by importing into other database applications such as FileMaker, MySQL, MS-SQL, Postgre, Oracle, FoxPro, Caché, etc. using the provided CSV files. In addition to the complete NPPES data, specialty and subspecialty descriptions have been added to the taxonomy data, enabling searches for "dentist," "otolaryngology," etc., in addition to the provider taxonomy codes Specialties are reported by providers using their Provider Taxonomy Codes. CarePrecise datasets include both the taxonomy code and the specialty in plain English. Click the link to see current specialties.. The LEIE database of providers excluded from billing Medicare is included and linked to NPI records using advanced record-linkage, providing an NPI number for LEIE (barred provider) records on matching providers. An "InPECOS" field indicates all providers who have completed the enrollment process for billing Medicare and are currently in the federal PECOS system. The CarePrecise Access dataset can be used to:

- NPI number The US Department of Health and Human Services (HHS) requires every healthcare provider "covered entity" -- both individuals and organizations -- to obtain a National Provider Identifier (NPI) number by enumerating with the National Plan & Provider Enumeration System (NPPES). The Centers for Medicare and Medicaid Services developed and maintains this database. "Covered entity" is defined under under HIPAA as any provider who may be listed on any electronic healthcare claim -- essentially every U.S. physician, dentist, optician, etc., and every hospital, clinic, pharmacy, lab, and many other kinds of healthcare provider.

Group Practice Data
Two separate but complementary data sources now make it possible to identify group practices. The first, group data provided by CMS, identifies the practice record. We provide both a "Group" flag in the provider's data, and new relational table with the reported codes (tTaxoGroup).
The second tool we provide to identify groups is the co-location code table (tCoLoCode). The CoLoCode is an algorithmic derivation from provider practice data, which facilitates querying providers by practice location. Our SQUIRRelate™ record linking system standardizes and conforms location data beyond postal specifications to link providers practicing together.
These data can be used together to successfully identify physician groups and all of their specialties, and the CoLoCode data can be used to identify any provider type in a co-located group practice, including dentists, optometrists, radiologists, etc.
Example Group Practice Application: The co-location codes, exclusive to CarePrecise datasets, let you find large (or small) clinics, based on the Count field, which shows the number of providers in the database with the same practice location. For instance, while the code UICFLG70MMPNPNHRR seems meaningless, it happens to be a CoLoCode for the Cleveland Clinic, and it can instantly filter the database to show you all of the 2700+ providers practicing at the Cleveland Clinic's 9500 Euclid Avenue site.
FILE FORMATS
The product is delivered in two formats as downloadable files, in two file formats, each containing identical data:
- Microsoft Access 2000/2003/2007/2010 (.mdb); normalized relational tables, plus a control database with query examples
- Comma Separated Values (.csv); normalized relational tables

SYSTEM REQUIREMENTS
For operation as a Microsoft Access database, Access 2003 or later is required. CSV files can be imported into any ordinary database system such as FileMaker, MS-SQL, MySQL or SQL Server. You should have 2 gigabytes of RAM (4GB recommended). Approximately 1GB of disk space is required for storage of the MDB files (less for the CSV files), plus additional disk space when manipulating the data. The CSV files may be imported into any application that supports comma separated value files and supports large file sizes.
DATA STRUCTURE
CarePrecise Access is distributed as eleven normalized relational database tables (MDB files), each keyed by the National Provider Identifier (NPI) number, plus a core Microsoft Access database that links the ten files together and includes query examples, two tables that identify added and dropped provider records, and CSV versions of the files. Through data normalization, CarePrecise reduces the highly redundant 329-field NPPES flat file structure to 65 unique fields, removing large volumes of unused space while preserving every character of the data and all of the original granularity. The data is in no other way "compressed," but is ready to access at a fraction of the original file size, and in a form that can readily be searched, manipulated and imported into applications.

The NPI identifies the provider record, and the CoLoCode identifies the co-located group. The CoLoCode is an up to 40-character string that stands in for detailed location data, and is not intended to be sensical to the human viewer. Record linking algorithms are used to enrich our data when direct correlation (via a unique identifier field) is not available, using available information in name, address, phone, taxonomy, and other fields. In particular, the data on providers excluded from billing Medicare (from the HHS Office of Inspector General's excluded providers database) can only be linked to NPI records by this means. Please note that when matches are found, there may be instances where an individual or firm has the same or similar name and location information as an excluded provider but is actually a different party. CarePrecise data is licensed for use by the purchaser and may not be redistributed. Review the End User License Agreement. Redistribution licensing is also available. Unless a redistribution or multi-user license is obtained separately, the standard End User License Agreement
permits the licesee to use the data in any way desired as long as it is within and for the buyer's own organization and is used by only one user at a time (may not be installed on multiple computers for use by multiple simultaneous users). Alternativel licensing options are available for uses that involve distribution of the data or use of the data by more than one user and/or for public viewing, and are purchased separately.
