APPLICATIONS: CarePrecise Access Complete is a normalized relational database containing every healthcare provider and all of the data reported by the provider to the federal National Provider Identifier (NPI) database, plus additional data other healthcare provider data sources. The format is specifically designed to be used on standard office-class desktop and laptop computers, overcoming the filesize and maximum row and column issues posed by the federal NPPES database through a process that removes blank fields from the flat file and organizes the data into separate tables for name information, address information, etc., plus data from other federal sources. This allows queries to act on the full (originally approximately 4 gigabyte) dataset within a normal PC environment, either using the Microsoft Access mdb files, or by importing into other database applications such as FileMaker, MySQL, Postgre, Oracle, FoxPro, Caché, etc. using the provided CSV files. In addition to the complete NPPES data, specialty and subspecialty descriptions have been added to the taxonomy data, enabling searches for "dentist," "otolaryngology," etc., in addition to the provider taxonomy codes Specialties are reported by providers using their Provider Taxonomy Codes. The LEIE database of providers excluded from billing Medicare is included and linked to NPI records using an algorithmic matching algorithm, providing an NPI number for matchable LEIE records. NEW: Group Practice Data: Two separate but complementary data sources now make it possible to identify group practices. The first, group data provided by CMS, identifies providers who have reported a multi-specialty group or single-specialty group practice. We provide both a "Group" flag in the provider's data, and new relational table with the reported codes (tTaxoGroup).

Beginning with the December release (to be distributed in January, 2012), the second tool we provide to identify groups is the new co-location code table (tCoLoCode). The CoLoCode is an algorithmic derivation from provider practice data, which facilitates querying providers by practice location. Our SQUIRRelate™ record linking system standardizes and conforms location data beyond postal specifications to link providers practicing together.

These data can be used together to successfully identify physician groups and all of their specialties, and the CoLoCode data can be used to identify any provider type in a co-located group practice, including dentists, optometrists, radiologists, etc.

Example Group Practice Application: The co-location codes, exclusive to CarePrecise datasets, let you find large (or small) clinics, based on the Count field, which shows the number of providers in the database with the same practice location. For instance, while the code UICFLG70MMPNPNHRR seems meaningless, it happens to be a CoLoCode for the Cleveland Clinic in , and it can instantly filter the database to show you all of the 2700+ providers practicing at the Cleveland Clinic's 9500 Euclid Avenue site.

FILE FORMATS: The product is delivered in two file formats, both on one DVD, each containing identical data:

  • Microsoft Access 2003/2007/2010 (.mdb); normalized relational tables, plus a control database with query examples
  • Comma Separated Valued (.csv); normalized relational tables

SYSTEM REQUIREMENTS: For operation as a Microsoft Access database, Access 2003 or later is required. CSV files can be imported into any ordinary database system such as FileMaker, MySQL or SQL Server. You should have 2 gigabytes of RAM (4GB recommended). Approximately 700MB of disk space is required for storage of the MDB files (somewhat less for the CSV files), plus additional disk space when manipulating the data. Because the files are quite large, we recommend disk defragmentation prior to copying the files to your computer for best performance. The CSV files may be imported into any application that supports comma separated value files and supports approximately 3.5 million rows of data (more for taxonomy and other ID tables).

DATA STRUCTURE: CarePrecise Access is distributed as eleven normalized relational database tables (MDB files), each keyed by the National Provider Identifier (NPI) number, plus a core Microsoft Access database that links the ten files together and includes query examples, two tables that identify added and dropped provider records, and CSV versions of the files. Through data normalization, CarePrecise reduces the highly redundant 329-field NPPES flat file structure to 65 unique fields, removing large volumes of unused space while preserving every character of the data and all of the original granularity. The data is in no other way "compressed," but is ready to access at as little as 13% of the original file size (varies), and in a form that can readily be searched, manipulated and imported into applications. The source data (NPPES) permits group codes to be reported in up to 15 fields in this section. CarePrecise creates a 1-to-many table, listing each of a provider's reported txonomy group codes in a separate record. The Gnum is the NPPES Group Taxonomy field number in which the code was reported, allowing the code be matched with the specific group specialty in the tTaxo table. For instance, a code in tTaxoGroup field Gnum 3 indicates a group practice of the specialty reported in Tnum 3 in the tTaxo table.

 

This is the newest of the data released by CMS in the NPPES file. This is where the group taxonomy codes are reported. CMS does not report the codes in the taxonomy section (which appear in our tTaxo table) because they represent an additional layer of information on any one or more of the tTaxo codes.

Provider co-location code (algorithmically derived*). The NPI and CoLoCode comprise the dual relational key for this table. The NPI identifies the provider record, and the CoLoCode identifies the co-located group. The CoLoCode is an up to 40-character string that stands in for detailed location data, and is not intended to be sensical to the human viewer. The CoLoCode /Group Practice system is in beta and subject to change, although code assignment per location is designed to remain static month-to-month (i.e., a given practice group's code would remain the same from month-to-month, while members' codes would change as they move to different practice groups) unless the group's location changes, as the code is predicated on practice location.

 

This code is an algorithmically derived conformation of the provider's practice location, permitting identification of providers who practice at the same location, for instance, physician groups, dental groups, radiology groups, etc. By using this table in conjunction with the tTaxoGroups table, it is possible to visualize the group entity (generally an organization) and its provider members.

Although each provider should have only one primary practice location in our database, technically, the tCoLoCode table is 1-to-many, because our system may not be able to resolve a single practice location per provider. There may be up to two distinct CoLoCodes for some providers.

* Algorithmically derived data in the CPAC dataset includes data that is extracted/mined from the source databases, but which has a margin of error and should not be relied upon as conclusive. Record linking algorithms are used to enrich our data when direct correlation (via a unique identifier field) is not available, using available information in name, address, phone, taxonomy, and other fields. In particular, the data on providers excluded from billing Medicare (from the federal LEIE database) can only be linked to NPI records by this means. In like manner, the available federal data does not provide information on providers' practice groups; using record-linkage technology, we can extrapolate the groups assuming that providers sharing the same practice location are members of the group reported at that location, and these group data can be corroborated or enriched with the group practice codes provided by CMS.
