Types of Historical Data We Can Provide

CarePrecise maintains historical healthcare provider data available in four different forms, with different starting dates for different formats, through the current monthly release. Pricing is based on the volume of the order, and in some cases on the age of the data.

CarePrecise archived data Basic Historical Information in our Standard Products

Abbreviated information on deactivated providers going back to February 2011 in the delta database is included with purchases of CarePrecise Access Complete, Master Bundle, CarePrecise Gold, and CarePrecise Platinum. Also included with those packages in the Dropped records table, that gives the provider's last information available before removal of their record from the NPI registry. Note that records that have been deactivated in the NPI registry can be reactivated. Reactivation dates are shown in the delta database, NPPES_Delta.mdb. For detailed descriptions of the included data, visit the documentation page and search "tDropped" and "NPPES_Delta" using your browser's CTRL+F (Find) feature. These data are for deactivated providers; if you need the full picture of month-to-month changes in active provider records, see the archival offerings below.

 

CarePrecise archived data Physicians' Sanctions, Reinstatements, Deactivations and Reactivations (SRDR) Database

Monthly sanctions/reinstatements from November 2014. Monthly NPI deactivations/reactivations from May 2005.

The data includes 2.5 million records on physician sanction and deactivation activity. The SRDR is an ongoing-change-tracing database, tracking physicians only. Sanctions include the reasons and dates; reinstatements include the date the provider was discovered. Dates of initial enumeration, deactivation and reactivation in the federal NPI registry. This data is used by health plans and their contractors, governments and law enforcement, in audits and fraud investigations, and by potential employers. The SRDR provides visibility into when physicians were and were not authorized to receive reimbursement from health plans including Medicare, and reasons for their exclusion from billing privileges. Data is "slice of time" as it existed historically agnostic to dispute. It is sourced from public records and is not subject to edits or opt-outs on the part of clinicians.

Two-year subscription to ongoing monthly SRDR updates (includes complete historical SRDR): $12,990

CarePrecise archived data Archived Production Databases

Monthly production archives from February 2011.

This archive includes primary CarePrecise databases archived monthly, and includes the backend format of each particular data set. CPAC and EPGH data sets are in Microsoft Access 2007 and later .ACCDB format. The Authoritative Hospital Database (AHD) and the Authoritative Physician Database (APD) are in Microsoft Access .MDB format. Packs include the data as it was archived, according to the particular protocol in place at the time of the release.

Files are available in both .CSV (Comma Separated Values format) and .MDB (Microsoft Access Database pre-2007 format compatible with current Access versions).

Pricing is based on the number of monthly release packs in your order, and the age of the data, with a three-pack (three monthly releases) minimum order:

  • The 3 most recent months prior to the current release (all include CPAC, EPGH, AHD, APD): $990 per pack
  • The 4th most recent through 12th most recent months (include CPAC, EPGH, AHD; months since November 2019 include APD): $800 per pack
  • The 13th most recent month and beyond: $500 per pack

An upload fee of $80 per order is added unless files are ordered on physical media. A minimum media and shipping charge of $50 applies to physical media orders, and varies by media type and shipping cost. See media shipping options here. NPI Registry Archived NPPES Dissemination File

Monthly data releases available from September 2007.

If your need is for federal data on all of the HIPAA-covered U.S. healthcare providers going all the way back to September 2007, CarePrecise can supply any or all of the 150+ months of the National Plan and Provider Enumeration System (NPPES) files. The files are in Comma Separated Values (.CSV) format. You may purchase one of them, one month from each year, one from each quarter, or whatever your use case requires.

Pricing is based on the number of files in your order:

  • First 1 to 6 files: $100 per file
  • 7th to 18th files: $90 per file
  • 19th file and beyond: $74 per file

An upload fee of $80 per order is added unless files are ordered on physical media. A minimum media and shipping charge of $50 applies to physical media orders, and varies by media type and shipping cost. See media shipping options here.

Master Bundle Archived CarePrecise Master Bundle (CPAC and EPGH) Files

Monthly data releases from April 2017.

The Master Bundle includes the CarePrecise CarePrecise Access Complete products, plus the Extended Professional, Group and Hospital (EPGH) data files. The EPGH data links individual providers, such as physicians, with their practice groups, and includes their hospital affiliations, graduation year, and additional data.

Files are available in both .CSV (Comma Separated Values format) and .MDB (Microsoft Access Database pre-2007 format compatible with current Access versions). Products are in the same format and include the same data fields* as the current release.

Pricing is based on the number of monthly release packs in your order, and the age of the data, with a three-pack (three monthly releases) minimum order:

  • The 3 most recent months prior to the current release: $400 per pack
  • The 4th most recent through 12th most recent months: $300 per pack
  • The 13th most recent month and beyond: $200 per pack

CPAC packages Archived CarePrecise Access Complete (CPAC) Files

Monthly data releases from April 2017.

The CPAC files include the monthly CarePrecise CarePrecise Access Complete products, which include the data in both CSV and .MDB (Microsoft Access Database) formats. The CPAC product always included all of the FOIA-compliant data fields* in the given month's NPPES dissemination file from the Centers for Medicare and Medicaid Services (CMS), as well as a small number of additional data fields sourced from the List of Excluded Individuals and Entities (LEIE) file from the CMS Office of Inspector General, and the Medicare Ordering and Referring Report.

Data is available in both .CSV (Comma Separated Values format) and .MDB (Microsoft Access Database pre-2007 format, compatible with current Access versions).

Pricing is based on the number of monthly release packs in your order, and the age of the data, with a three-pack (three monthly releases) minimum order:

  • The 3 most recent months prior to the current release: $250 per pack
  • The 4th most recent through 12th most recent months: $200 per pack
  • The 13th most recent month and beyond: $125 per pack

Additional Information

CarePrecise keeps an ongoing record of monthly statistics, such as counts of physician records, hospital records, numbers of dropped records (shrinkage) and added records (growth), excluded (sanctioned, barred from billing federal agencies such as Medicare), number of PECOS-enrolled providers (ordering and referring providers), as well as numbers of updated records.

What's in the data?

CarePrecise Platinum merges more than 400 data points for each provider record, collected every month from multiple authoritative sources, creating a single database.
