The Physician Sanctions, Reinstatements, Deactivations and Reactivations (SRDR) product is a running activity database, updated monthly, showing dates that physicians first enumerated their NPI record, was deactivated/reactivated, were sanctioned, and when sanctions were removed. This data is used by health plans and their contractors in audits and fraud investigations, and provides visibility into when physicians were and were not authorized to receive reimbursement from health plans including Medicare.
The data is structured in two tables: Detail and Activity, as well as an Activity Type lookup table. The detail table contains basic information on all providers that have registered with the NPI registry (name, practice address, NPI number, enumeration date, entity type, primary specialty). The Activity table contains dated entries for original enumeration, deactivation, Sanctions, and removals of sanctions. Sources are the NPPES (National Plan and Provider Enumeration System - the NPI registry) and LEIE (List of Excluded Individuals and Entities) databases from the US Department of Health and Human Services and the Office of the Inspector General for the Centers for Medicare and Medicaid Services. A Microsoft Access application is included, making it easy to view the historical activity of any U.S. HIPAA-covered physician.
