CCV Number Definition
The CMS Certification Number (CCN) is a unique six-digit identifier issued by the Centers for Medicare and Medicaid Services to facilities that participate in the Medicare program. It is used as the unique identifier for hospitals, nursing homes, home health agencies, hospices,
The CCN was formerly known as the Medicare Provider Number, Medicare Identification Number, OSCAR Provider Number, or Provider Number. It was renamed in 2007 to avoid confusion with the National Provider Identifier (NPI).
Format Structure
First 2 characters: Identify the state or territory (or a two-position alphanumeric code as numeric ranges are exhausted). For example, state code 36 indicates Ohio, 33 indicates New York, and 45 indicates Texas. Listing of state code prefixes...
Middle and final characters: The remaining characters indicate the type of facility and serve as a unique sequence or counter for providers within that state. For instance, the third digit often designates specific care types (e.g., 0 for acute care, 13 combined for Critical Access Hospitals).
Purpose and Use
The purpose of the CCN is to provide a unique identifier to verify Medicare and Medicaid certification, track survey and certification records, manage cost reports, and link federal healthcare datasets, such as quality measures and hospital compare data.
Relation to NPI Numbers
While the NPI acts as the primary provider identifier for general claims, the CCN remains active for specialized certification, assessment, and regulatory tracking, providing a single unique number per facility.
CCN-to-NPI Crosswalk
CarePrecise offers a CCN-to-NPI crosswalk in its Authoritative Hospital database, rolling up medical facilities' NPI record numbers under each unique CCN number. To flesh out the NPI data for each hospital, linked to their affiliated physicians, add the CarePrecise Advanced dataset or CarePrecise Platinum.
The crosswalk is an exclusive CarePrecise data table, produced using QoRelate® master record linkage technology. The exclusive CarePrecise CoLoCode is an indispensable asset in linking records, representing a hyper-conformed version of provider practice location data. Together with phone, fax, taxonomy and other identifying data, records can be rolled up under organizations with a high degree of accuracy.
