Used across the industry by health plans, governments, research programs, and law enforcement, CarePrecise Access Complete (CPAC) is the complete list of all HIPAA-covered U.S. physicians, hospitals, nurses, dentists, laboratories, pharmacists and every other kind of healthcare provider in the United States, with specialty and contact information – including practice and mailing addresses, phone and fax numbers – updated every month. In an open format that can be used on ordinary desktop computers and laptops, CPAC delivers the provider data collected in the federal NPI registry (NPPES) The Centers for Medicare and Medicaid Services (CMS) maintains this National Plan & Provider Enumeration System (NPPES) database. All healthcare providers who are HIPAA covered entities must obtain a National Provider Identifier (NPI) by reporting their information via the NPPES, and are required to update it when it changes. This represents virtually all of U.S. physicians, hospitals, and other providers who bill insurance, write prescriptions, or are affiliated with a hospital. —the official federal NPI registry—updated monthly by the Centers for Medicare and Medicaid Services. Using QoRelate™, our advanced record-linking technology, we include providers' PECOS (Medicare) The federal database containing information on healthcare providers who are eligible to bill Medicare (Provider Enrollment, Chain and Ownership System). enrollment status, indicating providers who are eligible to bill Medicare, and warnings on providers posted in the federal List of Excluded Individuals/Entities (LEIE) List of Excluded Individuals/Entities (LEIE): The federal database maintained by the HHS Office of Inspector General containing information on providers with sanctions due to fraud, controlled substance conviction, neglect or abuse, and other infractions.
Up-to-date information
CPAC is comprehensive and authoritative, sourced from the information reported and updated as required by law by the providers themselves in their federal National Provider Identifier (NPI) record, and is the most accurate source of detailed information on the nation's 6.5 million doctors, dentists, nurses, clinics, hospitals, laboratories, pharmacies and all other provider types.
Historical data back 10+ years
Our separate database of rich data on deactivated providers since 2005 includes the date of deactivation, name, address, specialties - all of the same fields as the standard data on current providers. Updated and included with each month's release.
Multiple practice locations and alternative names
CPAC now includes two new tables:
- Other Practice Locations - multiple practice locations per provider
- Other Organization Names - alternative organization names per organization provider
Applications
CarePrecise Access Complete (CPAC) is designed to be used on standard desktop and laptop computers, overcoming the file size and maximum row and column issues posed by oversized data files, using a process that organizes the data into separate tables for name information, address information, etc., plus data from other federal sources. This allows queries to act on the full dataset (over 5 gigabytes) on a normal PC, either using the Microsoft Access database, or by importing into other database applications such as FileMaker, SalesForce, MySQL, MS-SQL, Postgre, Oracle, FoxPro, Caché, etc. using the provided CSV files. (Special licensing may be required for certain applications.)

FILE FORMATS
The product is delivered in two formats as downloadable files, each containing identical data:
- Microsoft Access (.mdb); normalized relational tables, plus a control database with query examples
- Comma Separated Values (.csv); normalized relational tables for import into other database software

SYSTEM REQUIREMENTS
For the CSV files: Any operating system and software that supports relatively large comma separated values files. For the MDB files: Microsoft Windows 7 or later, 4GB RAM, Microsoft Office Pro/365 or Microsoft Access 2007 or later.

CarePrecise Access Complete is a normalized relational database ready to use on ordinary office computers in Microsoft Office Pro or Office 365 with Access, each record keyed by the National Provider Identifier (NPI) number. Download a sample here.

LICENSING
CarePrecise data is licensed for use by the purchaser on a single computer and may not be redistributed. Review the End User License Agreement. Site licensing and redistribution licensing is also available. Unless a redistribution or site license is obtained separately, the standard End User License Agreement permits the licensee to use the data in any way desired as long as it is within and for the buyer's own organization and is used by only one user at a time (may not be installed on multiple computers for use by multiple simultaneous users). Alternative licensing options are available.
