DESCRIPTION AND APPLICATIONS

Relational healthcare provider databaseCarePrecise Access Complete (CPAC) is a normalized relational database. The format is specifically designed to be used on standard office-class desktop and laptop computers, overcoming the filesize and maximum row and column issues posed by oversized databases, using a process that removes blank cells from flat files and organizes the data into separate tables for name information, address information, etc., plus data from other federal sources. This allows queries to act on the full dataset (more than 5 gigabytes) within a normal PC environment, either using the Microsoft Access MDB files, or by importing into other database applications such as FileMaker, SalesForce, MySQL, MS-SQL, Postgre, Oracle, FoxPro, Caché, etc. using the provided CSV files. In addition to the complete NPPES data, specialty and subspecialty descriptions have been added to the taxonomy data, enabling searches for "dentist," "otolaryngology," etc., in addition to the provider taxonomy codes Specialties are reported by providers using their Provider Taxonomy Codes. CarePrecise datasets include both the taxonomy code and the specialty in plain English. Click the link to see current specialties.. The LEIE database of providers excluded from billing Medicare is included and linked to NPI records using advanced record-linkage, providing an NPI number for all LEIE (barred provider) records on matching providers, including providers not given an NPI number within the LEIE. An "InPECOS" field indicates all providers who have completed the enrollment process for billing Medicare and are currently in the federal PECOS system. Co-Location Coding

The CoLoCode is an algorithmic derivation from provider practice location data, which facilitates querying providers by practice location. Our QoRelate™ record linkage system standardizes and conforms location data beyond postal specifications to link providers practicing together.

These data can be used together to successfully identify providers in groups, and to identify any provider type in a co-located group practice location, including pjhysicians, dentists, optometrists, radiologists, etc.

 

FILE FORMATS

The product is delivered in two formats as downloadable files, in two file formats, each containing identical data:

  • Microsoft Access (.mdb); normalized relational tables, plus a control database with query examples
  • Comma Separated Values (.csv); normalized relational tables

 

DELIVERY

DATA STRUCTURE

CarePrecise Access is distributed as normalized relational database tables (MDB files), each keyed by the National Provider Identifier (NPI) number, plus a core Microsoft Access database that links the files together and includes query examples, tables that identify added, dropped and deactivated provider records, and CSV versions of the files. Through data normalization, CarePrecise reduces more than 330 highly redundant flat-file fields to less than 70, removing large volumes of unused space while preserving every character of the data and all of the original granularity. The data is in no other way abridged, but is ready to access at a fraction of the original file sizes, and in a form that can readily be searched, manipulated and imported into applications, all relationally linked by the unique NPI number.

Warning

From the Office of Inspector General's excluded providers database (LEIE), indicatying a sanction prevents the provider from billing Medicare. NOTE: Beginning with the May 2017 release, only those LEIE-listed providers tagged with an NPI number in the LEIE source data are flagged in the Warning field, and the algorithmically-derived flags will be discontinued. This is consistent with the now-ubiquitous adoption of the National Provider Identifier. Up until the May 2017 release, this field was algorithmically derived.*

The source data (NPPES) permits group codes to be reported in up to 15 fields in this section. CarePrecise creates a 1-to-many table, listing each of a provider's reported taxonomy group codes in a separate record. The Gnum is the NPPES Group Taxonomy field number in which the code was reported, allowing the code be matched with the specific group specialty in the tTaxo table. For instance, a code in tTaxoGroup field Gnum 3 indicates a group practice of the specialty reported in Tnum 3 in the tTaxo table.

 

This is the newest of the data released by CMS in the NPPES file. This is where the group taxonomy codes are reported. CMS does not report the codes in the taxonomy section (which appear in our tTaxo table) because they represent an additional layer of information on any one or more of the tTaxo codes.

Provider co-location code (algorithmically derived*). The NPI and CoLoCode comprise the dual relational key for this table. The NPI identifies the provider record, and the CoLoCode identifies the physical location. The CoLoCode is an up to 40-character string that stands in for detailed location data, and is not intended to be sensical to the human viewer.

 

This code is an algorithmically derived conformation of the provider's practice location, permitting identification of providers who practice at the same location, for instance, physician groups, dental groups, radiology groups, etc. By using this table in conjunction with the tTaxoGroups table, it is possible to visualize the group entity (generally an organization) and its provider members.

Although each provider should have only one primary practice location in our database, technically, the tCoLoCode table is 1-to-many, because our system may not be able to resolve a single practice location per provider. There may be up to two distinct CoLoCodes for some providers.

The Delta tables are contained in a separate database included in the CarePrecise Access Complete dataset, named NPPES_Delta. They contain the complete data on providers whose records have been deactivated by CMS for various reasons, including retirement, death, or for any of several infractions. These tables contain the same fields as the standard provider data tables as defined above for their namesakes, and are populated with the data from previous data releases, with the deactivation date populated in the tEntity_DEACT table's DeactDate field.
Additional Data Included with Extended Professional, Group and Hospital (EPGH)/CPAC Master Bundleand CarePrecise Platinum. Data is sourced from PECOS, Physician Compare and Hospital Compare databases, and is updated monthly. Provioder records are a subset of the CPAC records, with approximately 550,000 physician records and 400,000 other providers. The EPGH is not sold separately, and is available only as a bundle. No time stamp is provided for EPGH updates, and no historical data is included.

Only active providers (have appeared on a Medicare claim in the past twelve months) are included in the EPGH dataset (unlike the CPAC dataset, which includes all US HIPAA-covered providers). EPGH data is available for approximately 560,000 physicians, and 300,000 other professionals, and is growing. The hospital data in EPGH provides an unduplicated listing of hospitals and their highest priority NPI number, and is linked to each hospital's affiliated professionals/physicians and practice groups. EPGH practice group data makes it possible to see the multiple locations at which an individual provider practices.
